Access and print the Parent Acknowledgement and Agreement Sheet here.

Step 2

Carefully read through each document listed below (links to each form provided). Initial and sign the Parent Acknowledgement and Agreement Sheet (see Step 1) signifying that you have read and agree with each of the documents below. Return the signed form to the office along with the forms in Step 3 on the first day of school.

• Education Agreement
• Parent/School Agreement
• Parent/Student Technology Agreement
• Student Handbook
• Student Standard of Conduct
• Uniform Requirements

Step 3

Print, complete and return these forms on the first day of school.

• Annual Field Trip/Emergency Medical Form
• Authorization for Student Pick Ups (if not already submitted)
• Over the Counter Medications
• Photo Release

2024-2025 Re-Enrollment

PLEASE DO NOT HAND IN AT THE BEGINNING OF 2023 SCHOOL YEAR.

These forms are due Jan. 31, 2024 during re-enrollment. Not applicable for current 8th grade students.
